Sent to the beneficiary and/or Representative Payee after every 12 months of Ticket use and active status.

Qp2

TPR Provider Notice

TPR Provider Notice available through the Portal to a participating service provider when the beneficiary does not reply to the Q-2 and the provider is either an EN or a State VR agency that provides TPR information on beneficiaries.

F-1

2nd Attempt Report Form

Sent to the beneficiary when the earnings or education progress submitted from the beneficiary is insufficient to pass OR when the beneficiary does not reply to the Q-2, AND the beneficiary's Ticket is assigned to a State VR agency that does not provide Timely Progress information on beneficiaries.

F-2

Fail Notice

Sent to a beneficiary who does not reply to F1 OR does not provide sufficient evidence after receiving the F1 OR the participating service provider does not reply to the Qp2 OR provides insufficient evidence on Qp2.

H-1

SSA Review
Favorable

Sent to a beneficiary who has received the (F-2) Final Fail notice and has requested a review and submitted the necessary evidence for SSA to overturn the failure decision.

H-2

SSA Review
Unfavorable

Sent to a beneficiary who has received the (F-2) Final Fail notice and has requested a review. However, the evidence is insufficient to pass the review. SSA upholds the failure decision.

Re-entry After Failure
Favorable decision

Sent to a beneficiary whose Ticket is still assigned or in use with a participating service provider but has received the (F-2) Final Fail notice and has submitted evidence that he/she has met the requirements on a date after the review period has ended. The beneficiary has 12 months where he/she is exempt from Medical Reviews from the date of re-entry.

Re-entry After Failure
Favorable decision

Sent to a beneficiary whose Ticket is no longer assigned or in use with a participating service provider but has received the (F-2) Final Fail notice and has submitted evidence that he/she has met the requirements on a date after the review period has ended. If the beneficiary reassigns the Ticket, he/she will have 12 months where he/she is exempt from Medical Reviews from the date of re-entry.

F-3

Re-entry Fail Notice

Sent to a beneficiary who attempts to re-enter by showing that he/she has met the requirements on a date after the review period has ended. However, the evidence submitted is insufficient to pass the review.
